The actual incident part refers to when there were two bedroom parks released in the same week in H2H once.  I think the square was checked off in H2H8 by Tubiao and Fiera going up against each other which were pretty conceptually similar. 

 

There was actually one in R3 of MM1 with two Futurama parks in the same round:

@csw The first comment on "House of Happiness" explains it somewhat. Two identical ideas coming up in the same round of the contest, without prior knowledge of each other.
